Hypnotherapy Consent Form & Information

✨ Hypnotherapy Terms & Conditions

Hypnotherapy is a collaborative process that uses relaxation, visualisation, and focused attention to support personal change, stress reduction, and self-improvement goals.

Hypnotherapy is not a substitute for medical, psychological, or psychiatric treatment and the hypnotherapist does not diagnose, treat, or cure medical or mental health conditions.

The nature of the sessions: 

​

  • Sessions may include hypnosis, guided visualisation, coaching conversations, and related therapeutic techniques.

  • The duration of each session is approximately: 50 minutes.

  • Number of expected sessions: as discussed, individual to each client

  • Sessions are 1:1 ... due to the nature of the session, there can only be the client and the therapist in the room, no distractions, no children or other adults

  • sessions may be recorded for use at home by the client only

What hypnosis is NOT:

​

  • you cannot be made to do anything against your will or values

  • you will not lose control or consciousness

  • hypnotherapy is not mind-control

  • results cannot be guaranteed

Client Responsibilities:

 

  • Participate willingly and follow the guidance offered during and between sessions.

  • Communicate openly with the hypnotherapist about any concerns or changes in your condition.

  • Refrain from attending a session under the influence of drugs or alcohol.

Hypnotherapist Responsibilities:

  • Provide a safe, confidential, and supportive environment for all sessions.

  • Maintain professional boundaries and follow ethical standards of practice.

  • Respect client autonomy at all times.

Confidentiality: 

​

All information disclosed during hypnotherapy sessions is kept strictly confidential, except in situations where disclosure is required by law, including but not limited to:

  • If there is risk of harm to self or others

  • Court orders or legal mandates

  • Abuse or neglect disclosures 

Fees & Payment:

Session Fee: £60

​

Payment is due:

By Bank Transfer before each session

Cash at the beginning of each session

Cancellation Policy:

​

Sessions can be cancelled or rescheduled by phone, text or email.

I completely understand that life happens and sometimes appointments need to change. To keep things fair and sustainable, I ask for 24 hours notice for cancellations or rescheduling.

When less than 24 hours notice is provided the full session fee is charged.

Of course, if there's an emergency or something unexpected, please let me know as soon as you can and we can talk it through.

Consent Form:

By signing below you understand that:

  • Hypnosis is a natural, safe state of focused attention.

  • Results vary from person to person and cannot be guaranteed.

  • hypnotherapy may bring up thoughts, feelings or memories. I understand support will be provided, and I may stop the process at any time.

  • You may also stop a session at any time.

  • You remain in control and can bring yourself out of hypnosis whenever you choose.

Medical Disclaimer:

It also confirms that you have informed the hypnotherapist of any relevant medical or psychological conditions.

You understand the hypnotherapist does not:

  • Diagnose conditions

  • Prescribe medication

  • Provide medical or psychological treatment

 

If necessary, You agree to seek advice from your GP.
